Case Management Program

Targeted Case Management supports adults with serious and persistent mental illness and children with severe emotional disturbance, working collaboratively with individuals and their families to ensure they receive tailored care to meet their needs.

Targeted Case Managers provide guidance and support to help individuals access medical, social, educational, vocational, financial, and other services to improve their mental health. They assist with referrals to resources, services, and community support to promote recovery and help individuals achieve their personal recovery goals.
